It is often helpful to provide us with some essential information at these meetings to ensure the time we spend together is most productive for you, rather than purely an information gathering process. To improve the accurate capture of this information and, at the same time, reduce the time needed for this process at our initial meeting, we have prepared a series of questionnaires (see below). We would encourage you to complete the questionnaire(s) relevant to you and send them to us in readiness for our first meeting, even if you do not have all the information requested to hand at present.
